Tear trough filler typically lasts between 9 and 18 months. How long yours lasts depends on the product used, how much is placed and how quickly your body breaks down hyaluronic acid. The visible result fades gradually, and some filler can remain beneath the eye after the effect has gone.
How Long Does Tear Trough Filler Last On Average?
Tear trough filler, also called under-eye filler, lasts 9 to 18 months for most people. The change is visible straight away; the final result is judged at around two weeks once any swelling has settled, and the effect then holds before fading slowly.
The filler is a soft hyaluronic acid gel placed in the groove between the lower eyelid and the cheek. What the treatment involves, and what it costs, is set out on our tear trough filler page.
Does Under Eye Filler Last 6 Months Or 2 Years?
Both figures are quoted, because published timescales vary. Earlier research put the typical duration at 8 to 12 months, while a 2025 study of 155 patients in the Journal of Clinical and Aesthetic Dermatology found the improvement was still measurable at 18 months.
Part of the difference is what is being measured. How long the result looks its best, how long any improvement can be seen, and how long filler stays in the tissue are three separate things. The figure of 9 to 18 months describes the visible result.
What Affects How Long Under Eye Filler Lasts?
Several things can affect how long the result lasts, and they vary from person to person.
- The product used. Fillers made for the thin skin beneath the eye are soft and fine, and products differ in how long they hold.
- How much is placed. A small, conservative amount can fade from view sooner than a fuller correction.
- How quickly your body breaks down hyaluronic acid. This varies between people and cannot be measured in advance.
- Your anatomy. A deep groove, or lost volume in the upper cheek just below it, can make the hollow show again sooner.
Smoking and sun exposure are often mentioned as well, but their effect on filler cannot be predicted for one person.
How Does Under Eye Filler Wear Off?
It wears off gradually. The body reabsorbs hyaluronic acid slowly, so the hollow returns over a period of months and does not reappear overnight.
- First 2 weeks. Swelling settles and the final result becomes clear.
- 2 weeks to 9 months. The result is stable for most people.
- 9 to 18 months. The visible effect fades and the hollow slowly returns.
- After 18 months. The visible effect has usually gone, although some filler may still be present.
Can Under Eye Fillers Last 3 Years?
The visible result rarely lasts three years, but the filler itself can. Hyaluronic acid can stay in the tissue beneath the eye well beyond the point where its volumising effect has faded, and a published case report describes it persisting in the lower eyelids for five years.
This is why an under-eye treated years ago can still hold filler, and why puffiness sometimes appears long after treatment. If that sounds familiar, our guide to tear trough filler that has gone wrong explains what can be done.
Will Under Eye Fillers Eventually Go Away?
In most cases, yes. Hyaluronic acid is broken down by the body over time, although beneath the eye this can take much longer than the visible result suggests. Where it needs to be removed sooner, this type of filler can be dissolved, which is one reason it is the type used beneath the eye.
When Should You Have A Top-Up?
A top-up is worth considering once the hollow has visibly returned, not at a fixed interval. For most people that point falls somewhere between 9 and 18 months after treatment.
Because filler can remain after the effect fades, the area should be assessed first to see how much is still there. Adding more on a set schedule, without that check, is one of the ways an under eye becomes overfilled.
Is There A Longer-Lasting Alternative To Filler?
For some people, yes. Fat grafting uses a small amount of your own fat in place of filler and lasts longer. Where the concern is a bag and not a hollow, lower blepharoplasty is the procedure that removes it, and our article on what causes under eye hollows explains how to tell the two apart.

Frequently Asked Questions
Is Tear Trough Filler The Same As Under Eye Filler?
Yes. Both names describe filler placed in the tear trough, the groove between the lower eyelid and the cheek. The two terms are used interchangeably.
What Happens If You Never Top Up Under Eye Filler?
Nothing needs to be done. The result fades and the area gradually returns towards how it looked before treatment, allowing for the natural change that comes with time. Whether to repeat it is your choice.
Can You Make Under Eye Filler Last Longer?
There is no proven way to extend it. The product used and how your own body handles hyaluronic acid are the main factors. Following your aftercare instructions helps the result settle well, but it is not known to change how quickly filler is broken down.
